We welcome you to the ITPEU Annual Benefit Plan. We are pleased to present you with this booklet describing the Plan.
The Plan is financed entirely by employer contributions made in accordance with the collective bargaining agreements between the Industrial Technical and Professional Employees Union, AFL-CIO (ITPEU) and the Employers. The Plan complies with the Employee Retirement Income Security Act of 1974 (ERISA), as amended.
The administrator of the Plan is the Board of Trustees of the ITPEU Annual Benefit Fund. The Board of Trustees in turn has engaged a contract administrator, M.S. Administrators, Inc. d/b/a ERISA Systems, 216 North Avenue East, Cranford, New Jersey 07016, telephone # (908) 276-0800, to handle the day-to-day administration of the Plan. The Plan also has the following web site –WWW.ITPEUBENEFITS.ORG.
A summary of the Plan's provisions and a brief discussion of some general facts of interest to you are presented in this booklet. To make this information as clear as possible, we have attempted to write the booklet in simple, straightforward language.
Part I of this booklet is a section entitled "About The Plan" which contains a general description of the Plan and important information regarding the way in which the Plan is administered. Part II of the booklet is a section entitled "Summary Plan Description" which describes the benefits provided by the Plan, eligibility for such benefits and how benefits are paid.
In describing your Annual Benefit Plan in brief form, however, it is not possible to explain each and every detail. Accordingly, Part III of this Booklet contains the official text of the Plan Document which provides a complete statement of your rights under the Plan.
The benefits described in this booklet are in effect on the date that this booklet is published.
We suggest that you share this booklet with your family since they may have an interest in the Plan. Keep the booklet for future reference and let members of your family know where this booklet is being kept.
